Individual Registration
To register as an individual please fill out the online web form below or fax the form to Madison Indoor directly at (859) 625.1383.

Directions for Online Registration
Please select the season in the "Season" dropdown menu, then choose the league you would like to register from the "League" dropdown menu. Then fill out all the other information and then click the "Submit Registration" button when you are done.

Team Registration

Fill out all the information on the form and return to Madison Indoor in person or by mail to 1136 Richmond Green Ct. Richmond, KY 40475.
