Membership Fee- $25.00 Annually
Early Registration $700 must be paid in full by October 2013 for Winter 1
Clubs bringing 5 teams or more will automatically qualify for early registration credit

STREET SOCCER LEAGUE
Begins Nov. 2013, Every Wednesday Night at 7:30pm. Cost is $40 per player for 6 week winter session. All fees must be paid prior to first week.

ADULT LEAGUES:
There are a variety of choices for our adults. Participate in men’s, women’s, under 35 co-ed, college night, over 35 league and Hispanic leagues.

All payments are due in full before the first game of the season

If you are an adult player please download the MI Adult Roster and Waiver form and return it to the facility completely filled out prior to game play. You can either email us at info@madisonindoor.com or fax Madison Indoor at 859-625-1383.
If you are registering a youth player please download the MI Youth Roster and Waiver form and return to the facility completely filled out prior to game play.
